Executing Strategies in a Global Environment Essay

Federal Express, known today as FedEx was developed by Fred Smith in 1973. Mr. Smith saw need for customer to receive packages sooner than 1-3 business days; this was the norm for express mail 40 years ago (Hill & Jones, 2013). Federal Express changed the structure of air cargo industry and improves on the overnight delivery service (Hill & Jones, 2013). In this paper, it will analyze value creation frontier of FedEx, determine which four building blocks will give competitive advantage to maintain profitability, its efficiency, and examine the overall competition the global industry (Hill & Jones, 2013). Analysis of Company’s Value Creation Frontier FedEx accomplished it value creation frontier with innovation. The founder saw that there was a need for faster and more reliable express delivery service. FedEx competitors offered 1-3 day deliveries shipments, but didn’t take into consideration residential customers or businesses that would need their shipments sooner. Mr. Smith saw this need, but it couldn’t be accomplished by sharing air space on commercial flights. On commercial flights passengers were first and the space that was left was divided amongst the competitors. For his vision to be achieved he needed his own fleet of cargo jets (Hill & Jones, 2013). Federal Express founder started with the higher end of competitive advantage which is differentiation. Mr. Smith invested $91 million dollars of venture capital plus $4 million dollar of inheritance funds, which he used to purchase an aircraft company in Little Rock, Arkansas. He started using the aircraft to deliver envelopes and small packages overnight within the United States (Ecommerce, n.d.). Mr. Smith didn’t give the official name Federal Express until 1973. He brought the operation closer to home, which was Memphis, Tennessee. The business was operating out of Memphis International Airport. By this time the company owned 14 Dassault Falcon Airplanes and he employed 400 workers (Ecommerce, n.d.). The next step for Federal Express was innovation. Mr. Smith launched a direct mail advertising promotion to increase the exposure of the company. With the government loosening the airline regulations, it made it permissible for federal Express to use bigger aircrafts such as Boeing 727s and take advantage of more flexible flight schedules (Ecommerce, n.d.). In 1979 Federal Express started using a centralized computer system  to track packages, routes, weather, vehicles, and employees. A short time after, a digitally assisted dispatch system was put in place for clients to electronically request pickups (Ecommerce, n.d.). For quality and excellence Federal Express understood the client’s expectation and started tailoring their service to fit those needs. For example; the bigger clients, they started housing merchandise to make shipments upon request. Federal Express held 43 percent of the delivery transportation market, in contrast to its rival UPS in the 1990’s (Ecommerce, n.d.). In order for Federal Express to maintain above average profitability, they should concentrate on two phases; which are efficiency and quality. Federal Express has five pillars for increasing profitability which are; making personnel functions and processes more efficient, update their air fleet, convert U.S. domestic network, increase international profits, and enlarge service offerings (Annual report, 2013). The focus for Federal Express is the customer. The customers’ requests are very important to the company’s strategy to operate focused systems which function independently, compete collectively, and are managed collaboratively. Because Federal Express unique model allows them to fine tune networks without compromising service to customers, their solution continue to be superior and they can act in response to marketplace changes quickly and efficiently (Annual reports, 2013). Maintaining Edge Rivals Product Differentiation Product differentiation is the development of designing products or services to satisfy the needs of the customers. Companies maintain a competitive edge over their rivals when it creates, makes, and sell products in the same industry (Hill & Jones, 2013). Federal Express has developed an express delivery service that is better than its rivals. Their service caters not only to big and small businesses, but also residential clients. Packages are getting to their destination faster, not just in the United States, but internationally. The ability to digitally track your package from first phase to the last phase of delivery give clients’ confidence that FedEx understand their needs and it builds trust (Hill & Jones, 2013). Capacity  Control Even though non-price competition helps established industries avoid cutthroat price cutting that reduces company and industry levels of profitability, price competition does occasionally occur when excess capacity exists in an industry (Hill & Jones, 2013). When a company like Federal Express cut prices rival companies will do the same thing out of fear the price cutter will be able to sell its complete inventory, while rivals will be left with surplus of goods. In so doing, this will lead to developing price wars (Hill & Jones, 2013). Federal Express would have to develop a strategy that would help them avoid this type of problem and stay ahead of its rivals. In most cases companies have two tactical choices; each can independently try to preempt its rivals and take hold of the initiative, or the companies must jointly find indirect means of coordinating with each other so that they are all aware of the shared effects of their actions (Hill & Jones, 2013). Business Model Efficiency Assessment Federal Express unique strategy works seamlessly and simultaneously on three levels. FedEx compete collectively by standing as one brand worldwide and speaking with one voice (http://www.about.van.fedex.com). The company operates independently by focusing on their independent networks to meet distinct customers’ needs and manage collaboratively by working together to sustain loyal relations with their employees, customers, and investors (http://www.about.van.fedex.com). Each division of FedEx has created its unique niche; yet work together as a whole. FedEx Corporation provides customers and businesses worldwide a broad portfolio of transportation, e-commerce, and business services. FedEx express invented express distribution and remains the industries global leader, providing rapid, reliable, and time-definite delivery to more than 220 countries and territories connecting markets that comprise more than 90 percent of the world’s gross domestic products within one to three business days (Investors, n.d.). There is still FedEx; ground, freight, and services. The different divisions have been established independently meeting the needs in today businesses, but work together in this vital industry to be  one voice (Investors, n.d.). FedEx also continues to stay ahead by quickly adapting to the changes in technology to provide better, faster service. This will cause clients to continually use their service and believe in their reliability. Impact of Global on New Business Strategy Federal Express is a well ran operation, which doesn’t need any change to the method of how it conducts business. The only suggestion that can be made is to improve on existing business strategy. FedEx is efficient on functions and processes of staff, having an efficient air fleet, and dominate in U.S. and international services. Just by strengthen of these strategies FedEx can be more profitable over a three year period (Investor, n.d.). Enacting numerous initiatives to improve their staffing efficiency and work processes, Federal Express would accomplish the streamlining of key processes to reduce the time and resources it would require (Investor, n.d.). Prioritizing important actions and remove or defer functions that have or become redundant due to technological, operational or market driven changes. Lately, generate greater efficiencies in the global sourcing program (Investor, n.d.). By FedEx up grading older aircraft with newer planes, there would be more cost-effective and fuel-efficient aircraft, the 757 and 767 programs will create a next-generation air fleet that to a large extent reduce their operating cost (Investors, n.d.). For example, based on a 750 nautical mile round trip, our trip costs will improve by: up to 20% for the B757 versus A310-200 and up to 30% for the B767 versus MD10-30 (Investors, n.d.). Applying new technology and processes will advance flight and crew scheduling. Adapting to pickup and delivery operations to maximize on road productivity will cause for quicker and reliable service (Investors, n.d.). Merging of facilities and stations will be more cost effective. Refining aircraft maintenance processes will improve fuel efficiency for their aircraft and vehicle fleet. Improvements like these can be done with no compromising the exceptional service quality their customers have come to expect (Investors, n.d.). With Federal Express expanding on their business; they capture growth from all parts of the global cargo market, including priority, economy, ocean and air forwarding, and critical inventory logistics focusing on (Investors, n.d.): Growing FedEx  International Priority and expanding FedEx international Transitioning FedEx International Economy shipments to third party line on certain lanes. Increasing freight forwarding and supply chain capabilities for new vertical like healthcare. Leveraging European expansion for greater densities and lower costs. With FedEx concentrating on delivering the solutions for the right customers at the right price, they will continue to strive for the right solutions in the market place. By so doing they may hold on to the majority of their annual rate increases, utilize the revenue management committee to optimize yield, market share, and profitability growth (Investors, n.d.). May have to drop less profitable accounts, and then develop on small to m edium customer base. Also leveraging residential revenue opportunities for their delivery managers, over time should allow for an even more dominate business (Investors, n.d.). Conclusion Federal Express has changed the face of the air cargo industry and overnight nights delivery service (Hill & Jones, 2013). The business model that FedEx put together for their company relied on the right strategy and the right people to implement the vision and the mission for the success of this company. When a business follows the strategy, makes adjustments as needed, is willing to take risk, and invest in technology; you have a formula for success.

Analysis of Shakespeare Sonnet 60

Like As The Waves Make Towards The Pebbled Shore Time is a common theme throughout Shakespeare's Sonnets, this is most apparent in Sonnet 60. This sonnet is about the ravages of time. How time never stops and is constantly changing. Also how time is aging us, and eventually takes what is has given us. But Shakespeare poetry will stand the test of time: Like as the waues make towards the pibled shore, So do our minuites hasten to their end, Each changing place with that which goes before, In sequent toile all forwards do contend. Natiuity once in the maine of light.Crawles to maturity, wherewith being crown’d, Crooked eclipses gainst his glory fight, And time that gaue, doth now his gift confound. Time doth transfixe the florish set on youth, And delues the paralels in beauties brow, Feedes on the rarities of natures truth, And nothing stands but for his sieth to mow. And yet to times in hope, my verse shall stand Praising thy worth, dispight his cruell hand. Sonnet 60 starts w ith a very relatable illustration of a waves constantly traveling towards the shore. This is like time in that there are minutes constantly, continuing, going to their end.Each minute or wave replacing the one that just happened, in a continuous march. Just like every wave is building in strength and then crashing again only to be followed by another in its place. Time cannot be stopped, one minute is always followed by the next in a never ending cycle. The second quatrain says that a new sun rises and with time it rises to maturity, noon, where the sun is its highest and king of the sky. Then the sun starts to set and now what once gave the sun its glory is now taking that glory back, time. This is a metaphor of a sun having a human life.The sun starts out being born â€Å"Nativity† and then crawls like a baby until it reaches its highest point where it is â€Å"crowned† with maturity. Then the sun continues to fall back to darkness or death. â€Å"And time that gaue , doth now his gift confound† this last line concludes the metaphor with the assertion that time both gives the gift of life and then takes in away. The final quatrain goes on to explain that time destroys the perfection of youth, and carves wrinkles in a beautiful face. â€Å"And delues the paralels in beauties brow. If you replace delues with deludes and beauties brow with our forehead, then you can see that its stating that times makes wrinkles or lines across your forehead. So, time is aging us. Times also feeds on the rarities of natures perfection, and lays waste to all in its path. â€Å"And nothing stands but for his sieth to mow. † Seethe is constantly used as a metaphor for death, this is saying that nothing stands in times way, or deaths. Again the metaphor of time giving you life and then taking it away is expressed in this quatrain, though it being much darker and showing how relentless and unforgiving time can be.This metaphor is also more relatable since it is about us and how time ages us and eventually leads to our death. â€Å"And yet to times in hope, my verse shall stand Praising thy worth, dispight his cruell hand. † These last couple of lines go on to explain that his verses shall stand the test of time, praising your worth in spite of time's cruel hand. These last lines are saying that even as time takes him, makes him old, and eventually even kills him, his poetry will live on, not affected by time's cruel hand. It looks like he was right because over five hundred years later and here we are today still reading and analyzing these works.Time is a very relatable thing, and this sonnet explains time very well. It explains what time is, it's just seconds building on minutes continually going to their end. Time is giving, giving someone life and power, raising that person to their prime. Time is also very cruel, it takes that power and life away from that person. Time is such a simple thing, it's only seconds and then m inutes, but through this sonnet it has been personified to something more, something greater. It is a giver and taker, it is life but is also death, and in the end it is time that takes us.

Murrow who, ironically, has no background of the profession but whose innovations opened doors and paved the way for the industry’s present prominence. Sao Feng was the Pirate Lord of the South China Sea until his death during the third film. He was bold man, albeit a bit immoral. He has a very weird body and a scary face that makes you scream when you first see it. The fifth character is Captain Hector Barbossa, who is the Pirate Lord of the Sea. He seemed to have an apple addiction, which was an interesting characteristic for a rum-soaked pirate. Barbossa has a monkey named Jack; this monkey travels with him all the time and seemed to have taken the place of a potential son. The last important character is Davy Jones. Thus, from the very beginning, Rubens intends the reader to understand that this is an interpretation of a relationship brought out through a glancing impression. Nothing is real or absolute but is instead an image developed in the speaker’s mind based upon ‘fragments’ that have been presented to her like burrs, which prick her painfully and refuse to let go. At the same time, however, he is accompanied to the office by a bag of â€Å"soft, yell ow apples† (3) and â€Å"a sense of being loved and laundered† (4). The alliteration used in the latter statement allows the words to flow smoothly, suggesting a blissful existence while the apples, a major symbol for the poem, remain soft and welcoming. The use of apples as a major symbol within the poem brings out another element of the negative within the positive. On the other hand, the blastocysts had been cultured in standard conditions. 2.2 Sildenafil Citrate purification from commercially available Viagra tablets In both the in vitro and in vivo experiments, sildenafil had been cleansed from commercially available Viagra tablets by leaving 20g sephadex G-25 overnight to swell in 100ml of distilled water. A column of 80ml was then applied with the sephadex gel and equilibrated in 100ml of distilled water at room temperature. A Viagra tablet of 38mg was then placed in 91.2ml of distilled water and slowly mixed with a magnetic stirrer at room temperature for a period of twenty minutes. It was then filtered for 20 minutes under a temperature of 4 d egrees Celsius. The liquid in which sildenafil has been dissolved was then applied on the column. The column was rinsed with the Viagra solution just before it was flushed with 400ml of water to wash away any possible small molecules. This way, only sildenafil was left precipitated to resin. The column was then applied with 1% Formic acid to rinse the sildenafil off. The absorbance of the rinsed solution was then observed and according to Francis et al. (2003), the rinsed sildenafil had a sharp peak absorbance at 40ml (Figure 3.1). Figure 3.1 Elution of pure sildenafil from sephadex column After sildenafil was rinsed, the column was washed again with 160ml or distilled water to eliminate the formic acid. After the column was free of formic acid, it was then washed with 320ml of 0.2% sodium azide in order to preserve it for later use. The rinsed sildenafil was then frozen at temperatures of -80 degrees Celsius. It was then dried by freezing it in a high vacuum. This was done by first sublimating the contents for a period of 8 hours at 0.37 mbar and under a temperature of -53 degrees Celsius, then desorbing it at 0.001 mbar for 3 hours. The weight of the crystallized contents was determined by examining the rate of absorbance of sildenafil over the whole sildenafil that had been eluted, assuming that there had been a recovery rate of 60%. Interpersonal communication is characterized by: communication from one individual to another, communication which is face to face and both the form and content of communication reflect the personal characteristics of the individual as well as their social roles and relationships (Ellis, 2009). Interpersonal communication develops relationships of some sort among the communicating parties for instance when there is high degree of trust among them, where each person is prepared to openly discuss their feelings and where the participants have a mutual liking toward each other (Hartley, 2005). In this case the kind of relationship created is that of teacher to student relationship. Both the teacher and the student have the responsibility of seeking clarification whether they understand each other to minimize conflicts between them. Interpersonal communication is always a two way process meaning that two parties must be involved. The parties pass messages to each other such that there is the sender and the recipient but this is not static since the sender also becomes the recipient to complete the communication process (Hartley, 2005). The geography teacher in this case acts as the source when teaching and the students as the recipients. On the other hand the situation can be reversed where the student becomes the source and the teacher the recipient, this happens when the student seeks clarification. Interpersonal communication is an ongoing process and not an event. However, during the process of communication several things may happen that may hinder or create problems such that the parties do not communicate as intended. In most cases these problems are termed as noise in the process of communication. They hinder the sender and the recipient from decoding the words and signs sent leading to misinterpretation of each other hence passing the wrong messages. For instance in the case where the geography teacher intends to communicate to students on probable examinable questions yet some other students do not understand the teacher and even though they are hardworking they end up failing the exam. In this scenario it can be concluded that some noise existed between the teacher and the students and the intended message was not communicated. The existence of communication problem between the teacher and the father of the student may be said to have resulted from anger of the father due to the son’s failure in the test and the information availed to the father by the son. The remedies to this interpersonal communication will also be discussed later in the paper. Problems of interpersonal communication Language barriers The complexities of language codes are often highlighted in communication across cultural boundaries. There are number barriers in interpersonal communication that are related to language. Among the most common include; lack of equivalent words where the sender and the receiver comes from different cultural backgrounds and they use same word to mean different things (Ellis, 2009), lack of equivalent grammar or syntax this may make the parties in the communication to misinterpret the information forwarded by each other. For instance same words may work as nouns or verbs or adjectives for example in English `lift a thumb’ or `thumb a lift’ in the first case the word a `thumb’ is a noun and in the second case a `thumb’ is verb. Use of idioms and similes may also cause misunderstanding as different culture use them differently. Pronunciation is also a major problem in interpersonal communication this may alter the meaning of the word for instance problems associated with pronunciation of `r’ and `l’ one may pronounce ‘right’ as `light’ (Bovee Thill, 2000). In a class context such pronunciation problems may make the students fail to communicate as intended. The language problems in the communications process are thus very common in classrooms for instance it’s common to find student having different class notes as they hear different things from their teachers. The language used by the geography teacher may have not been understood by the student who failed in the test; this creates a conflict between the student and the teacher and it’s escalated further to family of the student. Use of non-verbal signs Non-verbal communication is a type of communication which consists of unspoken cues that a communicator sends in conjunction with spoken or written message for example, a person’s tone of voice where the individual vary the tone of the voice by either making it loud or sharp, facial expression, eye behavior, head nodding, nose thumbing, thumb movement. In addition one’s postures and manner of walking also may have communication significance for instance a person may walk in a manner which indicates utter despair. Also the distance between two individual involved in private conversation shows the relationship between the two (Turner West, 2008). It is therefore the responsibilities of the receiver and the sender to decode the message passage so as to avoid misunderstanding. The receiver decodes and interprets the message sent and responds by sending back feedback which helps the sender to find out if the receiver has correctly interpreted the message. The problem arises where the receiver misinterprets the message sent since he or she will respond by wrong feedback. Such misinterpretation may arise since individual comes from different cultural background and a certain non-verbal communication may have a totally different meaning to other cultures, for instance nodding of the head in some culture symbolizes acceptance or agreement but in some cultures it symbolizes disagreement (Kalefleisch,1993). In a class context the teacher’s changes of tones, facial expressions or uses cues that they think will help them to communicate effectively. In hinting what topic will appear in the tests the expression made by the geography teacher may have been misunderstood by the students resulting in students revising in other topics which made them to fail the exam. Use of nonverbal cues at times thus poses a bid problem in interpersonal communication. Channel noise This problem is caused by physical barriers and is mostly due to our senses of smelling, touching, tasting, hearing and seeing. Among the common channel noise includes laughing talking, coughing, sneezing, and snoring such noise can be inside or outside the setting of the source and the recipient of the message. For instance an aircraft over a building or a lawn mower outside a classroom, people talking or yelling loudly in the hallways or outside windows it can be even murmuring of students in class in the class students noting may distract other from listening (Ellis, 2009). The speaker may also not be audible enough, may have inappropriate appearance or have bad mannerisms that contribute to poor communication with the audience. If the speaker keeps on jingling his keys from the pockets this is still noise that may distract effective communication (Campbell Nelson, 2010). A school has many activities going on at the same time for instance cleaning of pavements, students moving up and down to laboratories and respective classes, cutting or trimming of hedges outside the classroom or continuous murmuring or laughing of students in class. All these distract the student in one way or the other from the teacher hence causing the student to miss some of the teacher’s remarks. This means that communication is not effective. May be such noises distracted the student when the geography teacher hinted on the topic to appear in tests and this made the student not to capitalize on the hinted topic hence failing an exam which became the source of the conflict. Psychological noise/emotional problem It also presents barriers to communication. Nervousness and anxiety can sometimes in certain situations be barriers to communication as well as tension that may be caused by controversy or conflict. The source of the message may use words or phrases that may be viewed negatively by the other party. The topic of discussion may also be controversial or no appealing to the other party or the audience may feel that the speaker is biased hence he may lack interest in listening to the speaker (Hartley, 2005). On the other hand the topic of discussion may be boring and the person trying to communicate may also be boring this may make even the listener to start dozing or sleeping hence unable to hear the speaker’s main points. It also true that some people may not like the person trying to pass messages and so they pay little attention no matter the message intended to be communicated. At times psychological noise may result from the audiences habit of thinking of what is to happen next for instance may be they also have to address the audience or they have a test in the near future or the events that happened sometimes back. Fatigue, stress or sometimes wrong timing for instance too early in the morning, too close to lunchtime or too late in the day when the listener are tired and the listeners may be faking attentiveness hence not decoding the message (Huff, 2008). In class context for instance student fakes attentiveness to avoid being punished or to please their teachers. Such cases, the teacher may be cheated that the student have understood the message being passed to students yet this is not the case .The teacher leaves satisfied that the message has been passed. In our case the student who is failed to pass the test may had psychological problems when the teacher was explaining on the examinable topics in the tests hence failed to decode the message that was being passed on this left the teacher with the impression that all students were attentive and would pass exams if they capitalized on the taught areas but this was not the case since some students end up failing the test. In addition the problem of the communication between the teacher and the father of the student who failed the exam resulted from the irritation of the father due to his sons failure in the test, misunderstanding occurs when the father feels that the teacher has resp onsibility of making the student pass the test while may be the teacher felt that he communicated effectively to students on what to expect in the tests. Solutions to interpersonal communication problems Language problems should at all times is avoided when communication is taking place between individuals but the major concern is how to avoid them. The parties in a communication exercise should ensure that they use the words that have similar cross-cultural meanings so that misinterpretation is avoided. Use of idioms, similes and other figurative languages should also be avoided and if used they should explained and made sure that the recipient understands the message fully (Fussel Kreuz, 1998). Pronunciation problems can be avoided by the source only if he practices to pronounce words properly however in some contexts like in a class pronunciation can be solved by spelling the words used or writing on the board so that communication is made effective. In addition to these language problems the teacher should understands the language problems of his students so that assistance is given in special cases as the instincts of the teacher feel that they may not comprehend the terms used . Non-verbal cues are inherent in any communication process and people should not always assume that they understand what gestures means otherwise the fail to comprehend the message sent. In order to avoid the problems associated with nonverbal signs the recipient should pay attention to what is being said since most nonverbal cues are aimed to emphasize a verbal message. For instance in class context a teacher may raise the voice to underscore something previously said. In addition it should be made sure that nonverbal and verbal messages match to avoid miscoding and misinterpretation. People should also be tentative in interpreting nonverbal communication due to the cultural differences in nonverbal cues. It also advisable to avoid non-verbal distractions when communicating since it at times act as noise in interpersonal communication. For instance shifting your eyes or continuously playing with one’s hair may add other meanings in the message being conveyed. Non-verbal signs should also be put in context when using them. Attention should pay on non-verbal cues but they should place in the right context. To understand what nonverbal cues means one should consider the entire communication process not just on element of it and we also need to ask others about what certain nonverbal cues mean in their culture (Turner West, 2008). In order to avoid psychological problems in interpersonal communication it advisable that to ensure that the parties in the communication process are psychologically prepared and they understand each other situation to avoid conflicting communication. The parties in the communication process should ensure that their topic of discussion is relevant, not boring and well timed to avoid distractions to other issues. It’s also advisable that all parties are actively engaged for instance in class the teacher should engage students with questions to make sure that they do not fake attentiveness. In addition the case of parent teacher miscommunication, the parent should always be psychologically prepared that a student can fail or pass a test and it’s not always the fate of a teacher for student failure. To avoid psychological problem in interpersonal communication the parties should ensure that they know their feelings, analyses the situation, owning their feelings reframing w hen needed and empathizing such practices are complex activities that involve sensitivity, awareness, insight and empathy therefore just like any proficiency, emotional communication require patience and persistence (West Turner, 2008). Solution to the channel noise can only be rectified by making sure the right channel to communication is used. The parties in the communication process should ensure that conducive environment exists so that communication is effective. In this case internal settings of the class should have minimal noise only that cannot be controlled. The teacher should take the responsibility of ensuring source of noise such as murmuring is minimized; however little can be done on external settings since other activities must take place (Aswathappa, 2005). If in our case external noise is too high the teacher ought to raise his voice or the student to have asked the points missed for effective communication to take place. Conclusion Interpersonal communication is vital in developing relationships among individuals since every social interaction involves communication. As said earlier it involves at least two individuals passing messages to each other. Individual must be ready to express their feelings and emotions for interpersonal communication to be said to occur. It’s a cyclic process that is continuous from source to recipient and vice versa where the recipient becomes the source and the source the recipient for the process to complete. Interpersonal communicational though very important and inevitable in our day to day activities has certain problems that hinder effective communication to occur between individuals. It was originally developed and manufactured by Pierre Fabre Medicament in France, and was approved in that country as an antidepressant in 1997 [1]. It has since been approved for this indication in multiple countries and currently marketed for this indication in over 45 countries worldwide including several European countries. Cypress Bioscience bought the exclusive rights for approval and marketing of the drug for fibromyalgia purpose in the United States and Canada in 2003 from the manufacturer Pierre Fabre Laboratories [2-3]. In January 2009 the U.S. Food and Drug Administration (FDA) approved MIL only for the treatment of fibromyalgia, making it the third medication approved for this purpose in the United States [4]. Some of the drug information and properties are listed below: 2.1 Physical and chemical properties Chemical name : MIL is chemically designated as (1R,2S)-rel-2(Amino-methyl)-N,N-diethyl-1-phenyl-cyclopropanecarboxamide hydrochloride and its structure is shown in Figure 2.1. This activates 5-HT and NE auto and heteroreceptors culminating in a decreasing 5-HT and NE neuronal firing rates, synthesis, and release. On Chronic use MIL continues to block 5-HT and NE transporters without desensitization, but 5-HT and NE auto- and heteroreceptors are desensitized and thus, down regulated. Firing rates of 5-HT and NE return to normal, and the amount of 5-HT and NE released per nerve impulse is increased [7]. MIL has no significant affinity for ÃŽ ±- and ÃŽ ²-adrenergic, muscarinic (M1-5), histamine (H1-4), dopamine (D1-5), opiate, benzodiazepine, or ÃŽ ³-aminobutyric acid (GABA) receptors. MIL has no significant affinity for Ca2+, K+, Na+ and Cl– channels and does not inhibit the activity of human monoamine oxidases (MAO-A and MAO-B) or acetylcholinesterase [8-9]. One of the main differences between the various antidepressants and MIL is its equal preference and activity on the uptake of NE and 5-HT. The exact mechanism of the central pain inhibitory action and effectiveness in fibromyalgia symptom are unknown in Humans [10-11]. 2.3 Therapeutic Indications Treatment of depression Major Depression, also known as major depressive disorder or unipolar depression, is a highly debilitating disorder that has been estimated to affect up to 21% of the world population [12]. It is a CNS disorder characterised by a combination of symptoms that interfere with a persons ability to work, sleep, study, eat, and enjoy pleasurable activities [7,12]. Despite the advances in the treatment of depression with selective serotonin reuptake inhibitors (SSRIs) and serotonin and norepinephrine reuptake inhibitors (SNRIs), there continue to be many unmet clinical needs with respect to both efficacy and side effects. These needs range from efficacy in treatment resistant patients, to improved onset, to reductions in side effects such as emesis or sexual dysfunction. To address these needs, there are numerous combination therapies and novel targets that have been identified that may demonstrate improvements in one or more areas [12]. Management of Fibromyalgia Fibromyalgia (FM) is a complex syndrome characterized by chronic musculoskeletal pain which is often accompanied by multiple other symptoms, including fatigue, sleep disturbances, decreased physical functioning, and dyscognition. Due to these multiple symptoms, as well as high rates of comorbidity with other related disorders, patients with FM have a reduced quality of life. The reduced serotonin and norepinephrine levels observed in patients with FM suggest that medications which increase the levels of these neurotransmitters, such as serotonin and norepinephrine reuptake inhibitors (SNRIs), may have clinically beneficial effects in FM and other chronic pain conditions. MIL is an SNRI that has been approved for the management of FM [8, 13]. MIL was viewed as a wonderful new weapon in the fight against both depression and pain. Treatment of Lupus Recent studies proved that MIL is also useful against lupus. Lupus is a chronic autoimmune disease in which the immune system turns against the body and harms healthy cells and tissues. It is a rheumatic disease which can affect many parts of the body including the joints, skin, kidneys, lungs, heart or brain. Some of the most common symptoms include extreme fatigue, painful or swollen joints, unexplained fever, skin rashes, and kidney problems. Scientific evidence indicates that lupus is caused by a combination of genetic and environmental factors. Lupus is characterized by periods of increased or intensified disease activity, called flares [14-15]. Tolerability and side effects MILhas demonstrated numerous adverse reactions in human clinical trials with tolerability decreasing with an increasing dose. In the placebo controlled trials in patients with fibromyalgia, the most frequent spontaneously reported adverse events were as follows: nausea, palpitations, headache, constipation, increased heart rate and hyperhidrosis, vomiting, and dizziness [16]. Discontinuation due to adverse reactions was generally more common among patients treated with 200 mg/day compared to 100 mg/day. The adverse effects can originate from the fluctuation in the plasma drug concentrations of an active substance following administration and subsequent metabolism and/or elimination from the body. Hammered is a violent verb and its two syllables makes the word sound short and harsh. In the following line, ‘rain, guttering down’ this makes me think the guttering I have on my house, a purpose made moulded channel used to transport water. He deliberately used this word to convey just how much rain had fallen that it had naturally moulded gutters out of the mud, channelling the slime and slurry into waterfalls. There is also assonance in this sentence emphasising the guttering (which I have already analysed above). Wilfred Owen is cleverly able to relate to you a description of a bomb without ever actually calling it a bomb.
